About Wema Homes

We are a licensed residential facility under the Arizona Department of Health Services, offering behavioral health services for patients under 18 years old. Our care includes support for general mental health (GMH), serious mental illness (SMI), as well as substance and alcohol abuse.

As patient-centered home, we prioritize outcomes that are meaningful and valuable to each individual. Our mission is to equip patients with the skills and knowledge needed to sustain their mental health over time. By promoting healthy lifestyle changes and fostering growth and stability, our approach ensures the best opportunity for lasting recovery and a brighter future.

Admission and Readmission Criteria

1. Eligibility Requirements:

  • Age: Must be under 18 years of age.
  • Self-Care Capability: Able to manage self-care tasks, including self-administration of medications.
  • Willingness to Participate: Must be ready to engage in the behavioral health program.
  • Cognitive Functionality: Capable of functioning in a structured environment.
  • Medical Stability: No need for intensive ongoing medical care or 24-hour nursing supervision.
  • Documentation: Provide relevant court documents, if applicable.
  • Assessment and Planning: Completion of an assessment and initial treatment plan.
  • Physical Examination: Conducted within 30 days before admission or within 7 days post-admission.
  • Tuberculosis Screening: Evidence of being free from infectious pulmonary tuberculosis within 6 months prior to admission or within 7 days post-admission.
  • Intake Documents: Signing and completion of all intake documents, including necessary authorizations.
  • Appropriate Presenting Problem: 
    • Impairment due to a mental health diagnosis or behavioral issues.
    • A need for services as determined by the individual, guardian, treatment team, or court documentation.

2. Participation in Treatment:

  • Willingness and ability to establish and work towards mutually agreed-upon treatment goals and objectives.

3. Exclusion Criteria:

  • Individuals presenting an active danger to themselves or others will not be admitted. These cases will be referred to a Crisis Team and/or local law enforcement.

  4. Scope of Care:

  • The Administrator and/or Clinical Supervisor will ensure that the scope of services offered at Wema Homes aligns with the treatment needs of admitted residents.

Discharging Residents

 Wema Homes  support will coordinate with the resident’s clinical team and family supports to ensure ongoing care and treatment after discharge from the facility.

  Residents may be discharged from Wema Homes for various reasons, including but not limited to the following:

Voluntary Discharge:

  • Successful Completion: Completion of treatment as per the resident's goals.
  • Resident/Family Request: Discharge initiated by the resident and/or their family.
  • Higher Psychiatric Acuity: If the resident begins to exhibit psychiatric needs beyond what the facility can manage.
  • Needs Outside Agency Scope: Services required by the resident exceed the Agency's capabilities.
  • Beneficial Referral: Determined by the treatment team to be in the resident's best interest.

Involuntary Discharge:

  • Acts or Threats of Violence: Behavior posing danger to staff or other residents.
  • Substance-Related Violations: Selling, using, or buying illegal substances on facility premises.
  • Non-Compliance with Rules: Behavior resulting in negative impacts on other residents or staff.
  • High Psychiatric Acuity: Needs that are no longer consistent with the facility’s licensed services.
  • Medical Conditions: Medical issues requiring care beyond the facility's ability to provide.

Rights and Documentation:

   1. Grievance Process:

  • Residents involuntarily discharged must receive written notice of their right to file a grievance and a copy of the grievance policy and procedure.

   2. Discharge Order:

  • A medical practitioner or behavioral health professional must provide a documented discharge order unless the resident leaves against medical advice.

   3. Post-Discharge Referrals:

  • Residents will be referred to appropriate treatment or ancillary services (e.g., detoxification or opioid treatment services) as needed to support their transition.

This structured process ensures that discharges are handled safely and responsibly.
